By including the following items in your fax to our clinics, we will be better able to serve your patients. For your convenience, a fax form is included on this website that you can fill out and fax directly to our clinics:

• Your name, fax number, contact information
• Patient’s name
• Patient’s Date of Birth
• Patient’s Contact Information, including phone numbers, address
• Patient Social Security Number
• Patient’s Insurance Information
• If this is an on-the-job injury and a claim has been filed, please include that information
• Type of Body Injury
